The Centre supports patients at Princess Margaret Cancer Centre/UHN and collaborates with community healthcare providers across Canada.

As an integral member of the Wallace McCain Centre, the Clinical Coordinator supports the clinical operations, coordination and ongoing development of the program. The role is responsible for triaging and coordinating new patient referrals, facilitating the weekly multidisciplinary case conference (MCC), coordinating clinic schedules and clinical fellow assignments, and supporting timely, coordinated and patient-centred care.

Working closely with the multidisciplinary team, the Clinical Coordinator provides clinical workflow leadership and supervises dedicated administrative support to ensure effective clinic operations, continuity of care and a positive patient experience. The role also identifies opportunities to improve clinical processes, access, coordination and the overall patient journey.

The coordinator will work primarily within the McCain Clinic’s weekly multidisciplinary workflow, supporting coordination across medical oncology, surgical oncology, radiology, research and other clinical services.

Duties

  • Review relevant clinical information, including imaging, pathology, molecular profiling, and medical records, to support referral triage and coordination.

  • Coordinate next steps with medical oncology, surgical oncology, and research teams, as appropriate.
  • Coordinate patients throughout the referral and assessment process, including appointments, diagnostic investigations, referrals, and follow-up on outstanding items.
  • Maintain referral tracking systems and waitlists, monitoring referral status, outstanding investigations, and follow-up actions to support timely assessment and continuity of care.
  • In collaboration with the rotating physician, flag new referrals for potential clinical trial eligibility during the initial referral review and triage process, and communicate potential eligibility to the appropriate clinical trials team for further assessment.
  • Coordinate required diagnostic investigations for new clinic patients, including bloodwork, CT scans, MRI scans, and other tests as clinically indicated; facilitate ordering by appropriately authorized clinicians and ensure results are available for clinical assessment and multidisciplinary review.
  • Serve as a consistent point of contact for patients and referring providers, supporting communication and navigation throughout the McCain Clinic pathway.
  • Communicate referral status, next steps, and relevant clinical information with patients, referring providers, and members of the care team.
  • Identify and escalate urgent or clinically relevant concerns to the appropriate member of the care team and support timely follow-up.
  • Support preparation and coordination of patient cases for the weekly multidisciplinary case conference (MCC), including identifying appropriate cases, ensuring required clinical information is available, and documenting recommendations and outcomes.
  • Coordinate approximately 15 MCC cases per week, tracking cases through the referral, review, and follow-up process.
  • Maintain accurate, timely, and complete referral and clinical documentation.
  • Supervise and provide day-to-day workflow direction to the dedicated McCain administrative assistant, ensuring clinic, referral, scheduling, and MCC processes are coordinated effectively and administrative priorities are addressed.
  • Coordinate clinic schedules, maintain the clinical roster, and assign clinical fellows to clinic activities, ensuring appropriate scheduling, allocation of responsibilities, and coverage in collaboration with the clinical team.
  • Monitor and report program data and operational indicators related to referral volumes, triage decisions, access to care, MCC processes, and timeliness of assessment, identifying trends and opportunities for quality improvement.
  • Support adherence to established clinical workflows, program procedures, and guidelines, identifying gaps and coordinating follow-up actions to promote consistent care delivery, patient safety, and quality improvement.
  • Contribute to the ongoing development and improvement of the McCain clinical program by identifying opportunities to improve workflows, operational efficiency, access to care, and coordination across multidisciplinary services.
  • Promote a positive, patient-centred experience by identifying barriers throughout the patient journey and implementing or recommending improvements to communication, navigation, timeliness of appointments, and continuity of care.
  • Collaborate with the Director and multidisciplinary team to identify evolving program needs, address operational challenges, and develop and implement processes that support sustainable growth and high-quality patient care.
  • Act as an initial point of contact for operational concerns and communication issues involving patients, referring providers, and multidisciplinary team members, facilitating resolution where appropriate and escalating issues to the appropriate physician or manager as needed.
  • Perform other related duties consistent with the role, as assigned.
