Role Overview Provide comprehensive, patient-centered care focused on cancer screening, early detection, risk reduction, and tobacco treatment. Conduct patient assessments, evaluate cancer risk factors, and provide education and counseling on recommended cancer screenings and preventive health measures. Support patients through the cancer screening process, including appropriate follow-up and coordination of care. Provide tobacco treatment and support patients in developing strategies to reduce tobacco use. Collaborate with physicians, nurses, patient navigators, and other members of the multidisciplinary team to ensure coordinated, accessible care. Help promote cancer prevention and awareness within the communities served by the Ralph Lauren Center. Key Qualifications Physician Assistant (PA): Graduate from an accredited PA program NCCPA certification Active NYS license or qualified for state licensure Experience in cancer screening, preventive care, or risk reduction is preferred Experience with tobacco treatment counseling is preferred Bilingual fluency in Spanish is strongly preferred OR Nurse Practitioner (NP): Current New York State (NYS) RN and NP licenses/registrations Master's degree in nursing, prescriptive privileges (or eligibility) Related national NP certification Experience in cancer screening, preventive care, or risk reduction is preferred Experience with tobacco treatement counseling is preferred Bilingual fluency in Spanish is strongly preferred Core Skills Demonstrates a strong commitment to community health, preventive medicine, and improving access to healthcare. Skilled at building trusting relationships with patients from diverse backgrounds and providing culturally sensitive, compassionate care. Able to assess patient needs, identify risk factors, and make informed clinical decisions regarding cancer screening and preventive care. Communicates effectively with patients and families, providing clear education and guidance on screening recommendations, risk reduction, and tobacco cessation. Collaborates effectively with multidisciplinary teams to coordinate care and address barriers to accessing healthcare. Demonstrates flexibility and adaptability in a dynamic outpatient environment while managing competing priorities. Passionate about patient education, health equity, and helping individuals take an active role in their health. Additional Information Hours: Four 10-hour shifts per week, primarily 8:00 a.m.–6:00 p.m., with a rotating schedule that includes one Saturday every four weeks.
