Health Priorities Specialist - Patient Navigator - Evenings - Spanish Speaking Required Work as part of the ED health care team as a patient ally throughout the continuum of care. Coordinate the patient experience, from ED to after discharge. Then, go a step beyond to connect patients to care, including the specialist or service they need. Support patients to navigate the health care system, including telehealth, and up the ante on patient education for health issues seen throughout New York. The schedule for this position will be Sunday - Thursday from 3:00 PM - 11:00 PM.
