Job Description JOB SUMMARY The RN Case Manager is a collaborative process which assess, plans, implements, coordinates, monitors and evaluates options and services to meet the patient’s needs through communication and available resources to promote quality cost-effective outcomes. ESSENTIAL JOB RESPONSIBILITIES Gather all relevant data from chart review and patient interview. Objectively identify barriers, clarify/determine realistic goals and seek potential alternatives. Work with patient/family/physician/healthcare tam to develop a treatment/discharge plan to promote positive patient outcomes and cost effective care. Initiate and implement plan modifications secondary to treatment response through continuous monitoring and communication with the healthcare team. Actively promote communication between healthcare team members, patient/family, and physicians. Monitor and promote appropriate use of resources, patient status and level of care. Communicate with third party payors to ensure payment and maximize benefits. Support and educate patient/family, physician, and healthcare team on available resources and regulations. Represent the best interest of the patient and hospital by advocating for necessary funding, treatment alternatives, timelines and coordination of health services and re-evaluation of progress and goals.
