Position Summary:
As a POD Health Community Outreach Representative, you will be responsible for driving growth and expansion of POD Health’s services through provider-based and community referral development. This role focuses on building strong relationships with medical offices, senior-focused organizations, and community partners to generate Start of Care (SOC) opportunities for POD Health. You will serve as a key ambassador for POD Health — educating partners on our services, identifying eligible patients, and ensuring a smooth referral-to-enrollment process in collaboration with internal teams.
Key Responsibilities:
- Build and maintain relationships with, but not limited to: Primary care practices, Specialist offices, Community health organizations, Senior centers, and advocacy groups.
- Educate referral partners on POD Health services, eligibility requirements, and patient benefits.
- Drive POD Start of Care (SOC) volume by consistently generating and following up on qualified referrals.
Plan, coordinate, and host community and provider-focused events, including: Health education workshops, - Enrollment events, Senior center presentations, Provider lunch-and-learns
- Represent POD Health at community events, health fairs, provider meetings, and networking opportunities.
- Collaborate closely with intake, clinical, and operations teams to ensure timely follow-up and successful SOC conversions.
- Track and document all outreach activity, referrals, and follow-ups accurately in Salesforce within required timelines.
- Support POD Health growth initiatives, pilot programs, and special outreach campaigns as assigned.
- Maintain compliance with Medicare marketing guidelines and organizational standards.
Qualifications:
- Fluency in English and at least one Asian language is required
- Valid driver’s license and reliable transportation for local travel
- Community-based business development, Medicare or value-based care preferred
- Strong understanding of Medicare populations and community healthcare dynamics
- Proven ability to build provider relationships and convert referrals into enrolled patients
- Excellent interpersonal, communication, and presentation skills
- Cultural competency and experience working with diverse communities
- Proficiency in Microsoft Office and Salesforce CRM (or comparable systems)
