We provide a collaborative multidisciplinary team approach in delivery of family patient centered care. In this position, you'll be responsible and accountable for the care provided to patients in the clinic during each shift. You'll utilize the nursing process to assess patients, plan care, interventions and evaluate outcomes. You'll work alongside our physician team to assist with procedures and provide patient education. Our Outpatient Clinic team enjoys a great quality of life with weekday schedules only...no weekends or holidays. Key Responsibilities: Assesses comprehensive data including physical, psychosocial, emotional, cognitive, and spiritual needs and interprets assessment information to derive the diagnosis or issues as defined by the patient's condition. Involves patient, family and healthcare team members in formulating a cultural appropriate individualized plan of care and identifies expected outcomes based on assessment and diagnosis. Implements the plan of care in partnership with the patient, family and healthcare members. Ensures that all components of the plan of care are documented in the patient health record. Utilizes teaching strategies appropriate to the individual patient's condition and learning needs. Maintains a professional and therapeutic nurse-patient relationship and serves as a patient advocate assisting patients and families. Required Qualifications: BSN preferred. ADN candidates must be willing to achieve BSN within 5 years. BLS, ACLS, PALS Bilingual English/Spanish The pay range for this position is $49.03 - $69.22/hour. Compensation is determined based on years of relevant experience and departmental equity.
