Admissions Registered Nurse (RN) – Home Health & Hospice Elite Home Health & Hospice Elite Home Health & Hospice is seeking a compassionate and dedicated Admissions Registered Nurse (RN) to join our team in delivering exceptional home health and hospice care. As an Admissions RN, you will play a vital role in Keeping Our Community Home by guiding patients and families through the admission process, ensuring a seamless transition to care, and helping them access the services they need with dignity and compassion. What You'll Do Conduct admission visits for home health and hospice patients. Complete comprehensive assessments to determine eligibility and care needs. Educate patients, families, and caregivers on available services, the plan of care, and expectations for treatment. Coordinate referrals and admissions with physicians, hospitals, facilities, and community partners. Obtain and process physician orders and required documentation. Establish initial plans of care and facilitate a smooth transition to services. Collaborate with interdisciplinary team members to ensure timely and appropriate patient care. Maintain accurate and timely clinical documentation in accordance with agency, state, federal, and accreditation requirements. Serve as a trusted clinical resource for patients and families during the admission process. What You Bring Current, unrestricted Registered Nurse (RN) license. Home Health and/or Hospice experience preferred. Strong clinical assessment and critical thinking skills. Excellent communication, relationship-building, and customer service skills. Ability to work independently while managing multiple priorities. Valid driver's license, reliable transportation, and current automobile insurance.
