The Role
The Nurse works in collaboration with physicians, nurse practitioners, and other independent health professionals to provide patient-centred, quality primary health care services. Within the context of a patient-centred care model, and working within a multidisciplinary, inter-organizational team, the Nurse will:
- Provide nursing services, within defined scope of practice, to patients based on patients' assessments and treatment plans;
- Perform nursing interventions such as taking blood pressure and other vital signs, applying aseptic techniques such as sterile dressings, ensuring infection control, monitoring nutritional intake and conducting specimen collection;
- Administer medication and observe and document therapeutic effects;
- Monitor patients' progress, evaluate effectiveness of nursing interventions and consult with appropriate members of the health care team;
- Call patients for follow up and communication and review
- Maintain effective communication with all team members;
- Participate in multidisciplinary team meetings regarding planning, implementation and revision of patient care programs;
- Optimize patient safety;
- Maintain clear and concise patient care records in EMR
- Assisting other team members when needed
Must Haves
- Graduate with Degree in Nursing
- Current registration with the College of Nurses of Ontario.
- Current membership with the RNAO
- Previous related experience in a primary care setting is preferred.
- Ability to work collaboratively within an interdisciplinary team, as well as to work independently.
- Excellent verbal, written and interpersonal skills.
- Certification: ACLS, BCLS for Healthcare Providers with AED Training.
- Proficiency in the use of computers and electronic medical charting.
- Available to work evenings.
- Valid driver’s license, access to a reliable vehicle, willingness to provide home visits.
- Satisfactory Vulnerable Sector Record Check
