This is an opportunity for a strong clinical leader who wants to do more than manage day-to-day operations. You will have the opportunity to develop your team, strengthen clinical operations, build relationships with healthcare partners, and help advance a growing palliative care program within one of the Bay Area's established nonprofit healthcare organizations.

If you are looking for a leadership role where you can combine clinical expertise, team development, operational leadership, and program growth, we would like to hear from you.

About By the Bay Health
Our interdisciplinary approach addresses the clinical, practical, social, emotional, and spiritual needs of patients and families—with the goal of helping every person experience the best possible quality of life.

The Opportunity
As our Palliative Care Manager, you will provide clinical and operational leadership for the Adult Palliative Care Program and work closely with the VP of Palliative Care to strengthen and grow the program throughout the San Francisco Bay Area.
You will lead an interdisciplinary team while helping create the workflows, clinical standards, partnerships, and infrastructure needed to support the program's next phase of growth.

What You'll Do
  • Lead the day-to-day clinical operations of the Adult Palliative Care Program.
  • Coach, support, and develop clinical team members while fostering a collaborative and accountable culture.
  • Partner with the VP of Palliative Care on the strategic growth and development of the program.
  • Help design and implement clinical workflows, policies, protocols, and processes that support high-quality care and sustainable growth.
  • Coordinate patient referrals and ensure timely assignment, appropriate staffing, and continuity of care.
  • Monitor clinical quality, patient outcomes, documentation, and regulatory compliance.
  • Collaborate with hospitals, healthcare organizations, facilities, contracted partners, and referral sources to strengthen care coordination and access to Palliative Care.
  • Identify opportunities to improve the patient and family experience throughout the Palliative Care journey.
  • Participate in interdisciplinary collaboration and help solve complex clinical and operational challenges.
  • Support staff education, professional development, and clinical excellence.
  • Participate in the Administrative On-Call rotation.
Leadership Responsibilities
In this role, you will have the opportunity to directly influence both team performance and program development.
Responsibilities include:
  • Supervising and developing clinical team members.
  • Providing clinical leadership and mentorship to nursing staff.
  • Providing administrative oversight to interdisciplinary team members, as applicable.
  • Supporting staffing, workload management, and clinical coverage.
  • Creating an environment of accountability, collaboration, and continuous improvement.
What We're Looking For
We are seeking an RN leader who is comfortable balancing clinical excellence with people leadership and operational execution.

Required
  • Current, unrestricted California Registered Nurse (RN) license.
  • Minimum of two years of RN experience in palliative care and/or hospice within the past five years.
Preferred
  • Three or more years of supervisory or healthcare leadership experience within the past five years.
  • BSN preferred.
  • Experience leading interdisciplinary healthcare teams.
  • Strong communication, coaching, and relationship-building skills.
  • Experience improving clinical workflows, quality, or program performance.
  • Interest in helping grow and develop a community-based Palliative Care program.
Why Join By the Bay Health?

This role offers the opportunity to join an organization with a 50+ year history of serving Bay Area patients and families while helping build the future of community-based Palliative Care.

We offer:

  • Comprehensive benefits
  • Full-time, Monday–Friday schedule
  • Hybrid work environment
  • Mission-driven nonprofit organization
  • Collaborative interdisciplinary teams
  • Meaningful leadership responsibility
  • Opportunity to influence the growth and evolution of the Adult Palliative Care Program
  • The ability to make a direct impact on patients and families navigating serious illness
