Position Overview: We are seeking a compassionate and dedicated Nurse Patient Navigator to join our team supporting Classic Hematology. In this role, you will be an advocate, educator, and support system for patients. You will work collaboratively with a multidisciplinary team to ensure that our patients receive high-quality, comprehensive care throughout their treatment journey. Key Responsibilities: • Serve as an advocate, interpreter, educator, and counselor for patients in the patient population. • Manage the care of assigned patients, ensuring access to comprehensive healthcare and ancillary services. • Act as the primary point of contact for patients and their caregivers, providing guidance and support. • Maintain ongoing communication with patients and their families throughout the treatment process. • Utilize clinical nursing knowledge and skills in all aspects of care management.
