Maintain patient confidentiality and comply with agency policies and regulatory requirements. Demonstrate professionalism, reliability, punctuality, and effective time management. Work independently while collaborating with an interdisciplinary care team. Complete required training, meetings, and continuing education. 💙 What You'll Do Provide physical therapy treatments in patients' homes under the supervision of a Physical Therapist. Implement individualized treatment plans to improve strength, mobility, balance, coordination, transfers, gait, and overall function. Monitor patient progress and communicate changes to the supervising Physical Therapist. Educate patients, families, and caregivers on exercises, safety techniques, and assistive devices. Maintain accurate, timely clinical documentation. 🤝 Team & Communication Build positive relationships with patients, families, and colleagues. Collaborate effectively with the healthcare team to deliver exceptional patient care. Demonstrate compassion, professionalism, and respect for diverse backgrounds and lifestyles. 🚗 Work Environment & Physical Requirements Travel throughout the service area to provide in-home patient care. Ability to lift up to 40 lbs., assist with transfers and mobility activities, and navigate stairs and varying home environments.
