Collaborate with physicians, case managers, and interdisciplinary teams. Qualifications Current Oregon LPN License required. Graduate of an accredited nursing program. Current CPR certification. 1–2 years of acute care nursing experience. Home Health or Hospice experience preferred. Valid driver's license, reliable transportation, and auto insurance. Strong communication, assessment, and independent decision-making skills.
