Responsibilities:
- Accept and treat patients within your designated geographic area
- Provide physical therapy treatments to patients in their homes, as outlined in the licensed PT’s plan of care
- Monitor and report changes in patient status to the supervising PT in a timely manner
- Communicate effectively with patients, caregivers, and interdisciplinary team members to ensure coordinated, effective care
- Instruct patients, family members, caregivers, and Home Health Aides in exercises, safety awareness, and use of assistive devices as directed by the supervising PT
- Assist the PT in updating care plans based on patient progress and response to treatment
- Document patient treatments and observations accurately and promptly, in accordance with agency and regulatory standards
- Participate in case conferences and contribute to the care planning and discharge process, in collaboration with the PT
- Adhere to all federal, state, and local regulations as well as agency policies and professional standards of practice
- Physical Therapist Assistant Program Graduate
- Licensed Physical Therapist Assistant in the state of New York or eligible.
- Mandatory Certifications: First Aid, AED, & CPR Certifications.
- Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
- Ability to travel to patients’ homes within the assigned geographic territory
