LUX Infusion

Syracuse

Patient Referral Coordinator.

See your work in a new light.

At LUX Infusion, we’re reimagining infusion care to be more human, supportive, and connected. Inspired by lux—meaning light - we guide patients, providers, and partners through complex therapies with clarity, compassion, and confidence.

As a clinician-led, U.S.-based organization, we put people first - helping patients feel seen and supported through faster therapy starts, dedicated care coordination, and meaningful collaboration. Our commitment to inclusion, diversity, equity, and advancement (IDEA) is central to our culture, ensuring every team member feels valued and empowered to make a difference.

Schedule: Monday–Friday, 8:30 AM–5:00 PM EST (subject to business needs)
Location: Hybrid (Syracuse, NY) — 3 days onsite / 2 days remote following 90-day successful completion of onsite training
We’re looking for a Patient Referral Coordinator who wants their work to mean something — someone who brings care, accountability, and purpose to what they do every day.

Why This Role Matters

The Patient Referral Coordinator plays a vital role in ensuring patients are ready for care — clinically, administratively, and financially — before services begin. By coordinating referrals accurately and efficiently, this role helps remove barriers to treatment and supports a smooth, patient‑first experience.

In this role, you’ll contribute to work that:

  • Helps patients feel seen, informed, and supported as they begin care
  • Builds trust with clinicians and referral partners through accuracy and clear communication
  • Enables timely, compliant care delivery in a growing, clinician‑led organization

Everything you do connects directly to real people navigating complex healthcare journeys — and your work helps ensure that care can begin with confidence.

What You’ll Do

In this role, you will:

  • Ensure patient referrals are fully prepared for service by gathering required documentation, confirming insurance coverage, and securing authorizations in compliance with all regulatory requirements
  • Coordinate new infusion and enteral referrals from intake through initial processing, ensuring accuracy, timeliness, and clarity at every step
  • Collaborate closely with referral sources, hospital liaisons, pharmacy, nursing, billing, and branch teams to support seamless care delivery
  • Communicate coverage details and financial obligations clearly to patients or their representatives, ensuring understanding and agreement prior to service
  • Maintain accurate patient records, referral logs, and authorization tracking to support timely billing and reduce delays in reimbursement
  • Identify opportunities to improve intake and referral workflows while maintaining high standards of service and compliance
  • Bring reliability, follow‑through, and thoughtful problem‑solving to your daily work

What We’re Looking For

You may be a great fit if you bring:

  • A deep commitment to the human impact of your work and to supporting patients through complex processes
  • Strong communication and collaboration skills across clinical, operational, and external partners
  • Ownership and accountability in managing referrals, documentation, and deadlines
  • Comfort learning and adapting in a fast‑paced healthcare environment
  • A desire to contribute to a team that values empathy, accuracy, and high standards

Qualifications

  • Associate’s degree in Business, Healthcare Services, or a related field required
  • Minimum of one (1) year of healthcare experience required
  • Strong knowledge of payer requirements related to billing, authorizations, and coverage
  • Proficiency with computer systems and documentation accuracy
  • Strong organizational, multitasking, and problem‑resolution skills

Preferred

  • Two (2) or more years of experience with a home care or infusion provider
  • Prior home infusion experience on the provider or payer side

You don’t need to check every box — we care just as much about how you show up as what’s on your resume.
